What governs a Florida directive
Florida's healthcare-directive statute is at Fla. Stat. ch. 765. The official form is the Designation of Health Care Surrogate. Your directive is valid the moment it's signed under your state's witness and notary rules above; it only takes effect if your physician determines you can't make or communicate decisions yourself.
Living will companion in Florida

Who can witness your Florida directive
Fla. Stat. §765.202(1)-(2): the designation must be signed by the principal in the presence of two subscribing adult witnesses. The person designated as surrogate may not act as a witness to the execution of the document, and at least one witness must be neither the principal's spouse nor a blood relative.
