What governs a Arizona directive
Arizona's healthcare-directive statute is at Ariz. Rev. Stat. §36-3221 et seq. (§36-3201 contains definitions; §36-3221 is the execution statute; §36-3224 provides the sample form). The official form is the Health Care Power of Attorney. Your directive is valid the moment it's signed under your state's witness and notary rules above; it only takes effect if your physician determines you can't make or communicate decisions yourself.
